Above Knee Prosthetics / Trans-femoral Prosthetics

Pieter M Botha Orthotist & Prosthetist provides a wide range of prosthetic knees for Trans-femoral Amputees:  C-Leg Technology like the C-Leg The Standard of Care Prosthetic Knee, C-Leg Compact, the Genium Bionic Prosthetic System; Modular Knee Joints and Single-axis Knee Joints manufactured by Otto Bock.  The RHEO KNEE and POWER KNEE? manufactured by ssur are also discussed.  We also provide prosthetic liners for Trans-femoral Amputees.

C-Leg: Technology

See Video below :-  How Does the C-Leg Prosthetic Knee Compare with Other Knees?

C-Leg The Standard of Care

The C-Leg microprocessor-controlled prosthetic knee started a revolution for above‑knee amputees.  Manufactured by Otto Bock and introduced to the world in 1997, the C-Leg was the first prosthesis system to intelligently control and adapt to an individual?s gait. The C-Leg takes advantage of microprocessor-controlled hydraulics, which adapt dynamically to all walking speeds in real time. The revolutionary microprocessor that controls the C-Leg receives feedback from multiple sensors 50 times a second, making adjustments in real time.  The micro-processor can reliably secure the stance phase in both the C-Leg and Compact knees. The result is a system that recognizes which phase of gait the user is in?and reacts accordingly. This keeps your knee stable when you weight it, free-swinging when you step through?and always ready to support you if you stumble.  Users can speed up, slow down, take on hills and go down stairs step‑over‑step?all with a secure, comfortable stride.

C-Leg technology offers the advantages of permanent stance phase control, the ability to weight the prosthesis during flexion, dynamic alignment, less energy expenditure while walking, and relief for the sound side and the rest of the body.

Features

C-Leg from Otto BockTransformative technology
The revolutionary microprocessor that controls the C-Leg receives feedback from multiple sensors, allowing the knee to make adjustments in real time.

Always thinking about your security?so you don?t have to
The C-Leg offers tremendous stability?and flexibility. You can speed up, slow down, and go down stairs step-over-step with the assurance that the C-Leg is there for you.

Stumble recovery
Whenever the C-Leg senses that you?re in an insecure position?such as when you trip or stumble?it will increase resistance to provide the support you need to recover.

Multiple modes for multiple uses
The C-Leg offers two activity modes, set by your prosthetist for walking and other favorite activities, such as job tasks, golf, or riding a bike.

Stand at ease
A Standing Mode lets you lock the leg yourself using a specific motion pattern. With the leg flexed between 7 and 70 you use less energy, whether you?re washing dishes or standing in line.

Switch over
Switching between modes is as easy as bouncing on your toe?or a press of the wireless remote control.

The latest generation of C-Leg adds:

  • Improved swing phase control, users experience easier initiation at swing, with a smoother, more natural movement.
  • Higher weight limit300 lbs.  Benefiting even more users, including those who weigh more or who routinely carry or lift heavy loads. Genium? and C-Leg are rated for the highest weight limits in the category.
  • Improved stumble recovery, provides increased resistance in case of a trip or stumble.
  • A third mode, which gives you another choice, personalized for the user.
  • Adjustable safety mode. The default setting can be adjusted, personalized to the user?s preference.
  • Improved protection. New silicone caps covering the charging and adjustment jacks are now attached to the knee, securing the caps and improving protection from foreign matter and moisture.

C-Leg Compact

With the Compact version of the C-Leg, those with lower mobility and higher security needs can also benefit from the most important aspects of C-Leg technology. The Compact offers features such as unprecedented stability and a remote control that enables the user to lock the knee between 0˚ and 30˚ for long periods of standing.

C-Leg CompactFeatures

The Compact is designed for transfemoral prosthetic users who place a premium on safety and stability but won?t miss all the functionality of microprocessor swing phase control. This knee is excellent for the indoor walker who doesn?t take dramatic shifts in speed or direction. The Compact?s smooth responsiveness, reliability, and stance security is ideal for the person who moves more slowly, but requires the microprocessor stance control to maximize stability. The knee keeps stable when needed, free-swinging during swing phase?and always ready to support the user in case of a stumble.

Transformative technology
As with the C-Leg, the revolutionary microprocessor that controls the Compact receives feedback from multiple sensors, allowing the knee to make adjustments in real-time.

Always a secure step
The Compact offers tremendous stability. Whether you?re doing chores around the house or yard, you?ll step easily with the assurance that the Compact is there for you.

Stumble recovery
Whenever the Compact senses that you?re in an insecure position?such as when you trip or stumble?it will increase resistance to provide the support you need to recover.

Stand at ease
A Standing Mode lets you lock the leg yourself (using a specific motion pattern) between 0 and 30, helping you use less energy when you?re standing for longer periods of time. A vibration alert lets you know when the Standing Mode is engaged and disengaged.

Switch over
Switching between modes is as easy as bouncing on your toe.

Genium? Technology

The GeniumTM Bionic Prosthetic System is the result of extensive research and development efforts as well as over 14 years of practical experience with the C-Leg prosthesis system. A breakthrough in prosthetic knee joints, the Genium is based on a completely new technology platform, fueled by advances in control technology.

Genium Bionic Prosthetic System

The Genium? Bionic Prosthetic System

This state-of-the-art microprocessor-controlled knee joint utilizes a complex sensory system and sophisticated rule sets to mimic natural gait more closely than any other prosthetic knee. With multiple environmental inputs (including a gyroscope and an accelerometer), the Genium delivers unmatched functionality, including special features to help step over obstacles and ascend stairs.

The unique Genium technology is not a next-generation C-Leg, but is a sophisticated new technology platform built to gather exponentially greater microprocessor inputs that result in very precise responses.

For the wearer, motion becomes intuitive. Barriers and obstacles become an unconscious part of life instead of an interruption. Whether making a fast turn-and-grab to catch up with a runaway toddler, changing speed dramatically or backing up to navigate a crowd, or simply stepping over an obstacle instead of going around, the Genium makes it all easier without risking stability.

Optimized Physiological Gait (OPG)

OPG takes advantage of several important innovations to provide the most intuitive, natural walking available.

How Optimized Physiological Gait (OPG) works Features

Ascend stairs step over step
Stair ascension is possible due to additional sensory inputs, which allow the user to climb stairs step over step with more natural movements.

Cross obstacles more smoothly
Now everyday obstacles like curbs can be stepped right over?supported by a stable, flexed knee at landing? instead of having to swing the prosthetic leg around.

Walk backwards, forwards?any direction
Genium detects movement in any direction and releases stance independent of body weight, making walking in any direction much more secure.

Stand more easily
With its new Intuitive Stance Function, the Genium can tell when the user is standing and automatically resists further flexion. This allows for a natural distribution of body weight while standing, even on inclines and uneven surfaces.

Sit more naturally
If the user sits for more than 2 seconds (with the thigh parallel to the ground and minimal weight on the leg) the Genium reduces resistance to take a more natural position?and switches to an energy-saving mode!

Genium offers more freedom and more possibilities:

  • Splash-water resistant. No need to worry about a spray or two.1
  • Battery life of 5 days. More freedom, more peace of mind.
  • 5 additional modes. More choices of activities for the user.
  • Remote control. Easy and convenient.  Includes battery level monitor and step counter.
  • Inductive charging. Makes charging a snap ? and no open charging ports.
  • Built-in protector.  A tough cap at the knee avoids slipping and adds protection.
  • Knee angle up to 135 degrees so its easier to crouch down or kneel.
  • High weight limit of up to 330 lbs (150 kg). Now larger users and those who often carry or lift heavy loads can also benefit.
  • A more compact size. Shorter users and those with longer residual limbs are able to take advantage of this technology.
  • Computer Assisted Alignment (CAA). CAA helps the prosthetist more quickly align the prosthesis by calculating and visualizing the forces acting on the system and making adjustment and component recommendations. A groundbreaking innovation for fitting success.

Modular Knee Joints

The function of a mechanical knee joint is vital to the overall performance of a transfemoral prosthesis.

We improved this proven, unique construction ? which has proven itself thousands of times ? with this new generation of the 3R60.  The 3R60?s EBS securely provides up to 15 knee flexion at heel strike. This helps avoid hip hiking and other compensatory movements during the gait cycle. The knee is easily adjusted to suit the user?s individual needs.

Optimized Swing Phase Control

An innovative hydraulic system controls the behavior of the knee joint during swing phase. The dampening values of the hydraulic system are based on gait analysis investigations. For the user, this mainly means that he or she can easily initiate swing phase and use a wide range of walking speeds. Flexion and extension dampening can be individually adapted to fit the user.  The new generation of the 3R60 is smaller and at just 845 grams, also lighter than its predecessor. Four proximal connections are available for the knee so that you can easily fit different amputation levels.  The design of the 3R60 received the reddot award ? an international seal of quality for design ? in 2005.

The compact and dynamic single-axis modular knee joints 3R95 and 3R95=1 are more than a lightweight option. Their low-profile design makes them a perfect option for patients who wear a cosmetic cover and are sensitive to appearance.

The novel miniature hydraulic system is the key, creating dynamic movement resistance while supporting the needs of an active user. Swing phase is easy to initiate, which reduces energy expenditure. At high walking speeds, the degree of flexion is automatically reduced. Stance phase is secured by posterior placement of the joint and active control by the user.

Easy Adjustment

The two modular knee joints 3R95 and 3R95=1 have separate adjustments to fine-tune flexion and extension resistance to precisely control swing phase.

The design of the hydraulic linkage geometry carries powerful resistance even with a high degree of flexion. Optimized terminal dampening ensures a soft transition to the flexible stop, so that you can set the extension resistance to a low value without being counterproductive.

3R95 and 3R95=1

The 3R95 with a knee flexion angle of 135 is suitable for patients who weigh up to 330 pounds / 150 kilograms.
The 3R95=1 offers a knee flexion angle of 155 and is for users who weigh less than 165 pounds / 75 kilograms.

The strengths of the 3R95 are especially noticeable when it is combined with dynamic prosthetic feet.

BIONIC TECHNOLOGY BY SSUR

Introduction

Bionics is the application of biological principles to the study and design of engineering systems, especially electronic systems.  ssur's intelligent structures contain sensors, artificial intelligence and actuator technology which can respond in a human-like way to counteract loads, reduce vibration, change shape and enhance fluid and natural motion.

Sophisticated sensor technology mimics the body?s mechanoreceptors, providing artificial proprioception (a sense of where the limb is in space) by tracking factors that include load and angular displacement up to a rate of 1600 times per second.  Patented forms of artificial intelligence (AI) process information from the sensors and instigate the most appropriate  response for the next step.

The AI transmits a constant stream of signals which instruct high-precision actuator technology to act and deliver optimal function. The addition of powered motion represents a significant step in human energy conservation, and the ability to deliver the required response. A motor and battery generates appropriate prosthetic function, providing the precise amount of power necessary.

RHEO KNEE

Rheo KneeThe RHEO KNEE is a self-learning prosthetic system, so smart that it constantly learns and adapts itself to the individual and his/her environment. Advances in Bionic Technology by ssur have produced a device that automatically listens to your needs and thinks for itself. It learns the way you walk and recognises and responds immediately to changes in speed, load and terrain. Simple to operate, it is the most adaptable solution of its kind.

The RHEO KNEE uses advanced sensor technology in the form of load cells to sample force measurements at a rate of 1000 times per second. This means that the microprocessor in the knee is always aware of how the user is loading the prosthesis and can immediately identify different phases of gait or changes in load - when the user picks up a bag for
example, or the terrain beneath their feet alters suddenly. If anything out of the ordinary occurs, stance support is instantaneously activated, protecting the user from a potential stumble or fall. Angulometers are also used to sense the trajectory of the knee during swing phase. Resistance is adjusted automatically to generate a natural and smooth pendulum
motion, which adapts to different walking speeds or even external factors such as wind or the weight of a shoe.

Information from the sensors is fed through to the brains of the RHEO KNEE. The softwarebased artificial intelligence (known as the Dynamic Learning Matrix Algorithm?, or DLMA for short) does the thinking for the user, assessing incoming data and immediately initiating the most appropriate response. From the moment the first step is taken, the RHEO KNEE starts to learn an individual?s walking style, a process that continues over time, providing increasingly natural movement. It adapts to any situation, not just those within pre-set and limited parameters, enabling the individual to quickly regain confidence in
his or her ability to walk where and how they choose. And as the range of walking speed and activity increases, the RHEO KNEE keeps pace, providing virtual prosthetic adjustments to ensure that walking style is maintained with each step.

From firm, unyielding support when standing, to light and free movement when turning a corner or walking in confined spaces,
the RHEO KNEE delivers an exceptionally smooth and fast response. At the heart of the system is an advanced actuator which uses magnetorheological (MR) fluid and rotary blades to vary the knee?s resistance throughout each and every step. The MR fluid contains miniscule iron particles. When a magnetic field is applied, these particles start to form chains and resistance increases. The stronger the magnetic field, the stronger the resistance. Unlike hydraulic systems, the resistance offered by MR fluid is customised, activated only when the individual needs it and able to adjust at the speed of light. The result is minimal fluid drag within the knee, which restores a more natural pelvic position during pre-swing and reduces fatigue levels.

POWER KNEE?

Power KneeThe world?s first active prosthesis for above-knee amputees, offering unprecedented function and performance.

  • Provides the power to walk further and for longer.
  • Assists with the upward motion required for stairs and inclines.
  • Learns and responds to unique patterns of gait for natural, efficient motion.

The POWER KNEE works as an integrated extension of its user, restoring the power of lost muscles and the symmetry of movement so vital for those with limb loss. This highly intelligent, second-generation product is now lighter, smaller and even more efficient than before.

Enhanced sensor technology now located solely within the knee combines accelerometers, gyroscopes, a torque sensor and load cell, all of which work together to monitor the position, velocity and movement of the knee. They also measure with great accuracy the forces and load applied to the knee at any given moment. The sum of this data is fed through in a constant stream to the onboard computer within the knee, w hich, by utilising the power of artificial intelligence, can then work out exactly what type of activity the user is engaged in and successfully predict the next
intended movement.

Artificial intelligence (AI) located in the central micro processor acts as the ?brain? of the POWER KNEE, replacing the function that the body?s central nervous system would normally play in processing information about changes occurring inside and outside the limb if it was still intact. AI allows the knee to think for itself, to automatically process the data gleaned from the sensors and to anticipate and initiate the required function. The AI in the POWER KNEE also assesses gait symmetry in order to stimulate highly efficient gait patterns, which help to minimise strain on the back, hips and sound leg.

The electromechanical actuator in the POWER KNEE provides the power for active motion. As fast as information about the
user?s walking pattern is being processed, the microcontroller transmits a stream of signals which instruct the actuator on precisely how to act and the exact amount of power required. The actuator delivers a range of responses, including active lifting power, to lift the prosthetic leg up as the user ascends stairs or slopes; resistance for downhill slopes; and gentle forward propulsion, for improved gait dynamics on level ground. Additionally, an integrated torque meter helps recreate the
natural feeling of bounce when the leg comes into contact with the ground, absorbing shock and helping to protect back and hips.
